The Request Pertaining to Military Records (Standard Form 180) is the official form used to request verification and/or replacement of awards information from military records. SF-180 must be filled out completely and signed. Please ensure your request is as detailed as possible.
Examples:(1) Request complete verification of all awards entitled;(2) Request verification and replacement of all awards;(3) Request verification of entitlement to Combat Action Ribbon; or (4) Replacement of Purple Heart certificate.

Important Need to Know Facts:
Veterans and/or Next of Kin are only entitled to a one-time replacement of awards. This Headquarters does not replace foreign awards or marksmanship badges. Subsequent to the one-time replacements, Awards may be purchased from xxxxx Memorabilia Firms (include list)
Next of kin includes widow or widower, each parent, child, step child, child through adoption, brother, half-brother, sister and half-sister. Note:
If a CE&S award listed on the Veteran’s DD-214 cannot be substantiated it will be deleted from DD-221.
Marines discharged from the Marine Corps with an “Under Other Than Honorable Conditions” characterization of service. Title 10 U.S. Code Section 6249 prohibits the issuance of any medal, cross, or bar, or associated emblem or insignia to any person or his representative if his service after he distinguished himself has not been honorable are not authorize to receive any additional awards other than what is listed on their DD-214.
Incarnated Veterans may request verification of awards, however replacement awards are prohibited.

Below are the three different locations a Marine’s medical records could be located. If the Marine was seen or being seen by the VA they can check with the last listed location for those specific VA Claims medical records.
Medical records for Marines discharged from active duty or from the active/inactive reserves prior to January 1, 1994 should be in the possession of the National Personnel Records Center, (Military Personnel Records), 1 Archives Drive, St. Louis, Missouri 63138. That agency can also be contacted via their website at http://www.archives.gov/st-louis/ or by calling (314) 801-0800.
Medical Records for Marines who served between January 1, 1994 and December 31, 2013 should be on file at the Department of Veterans’ Affairs and can be obtained by writing to Records Management Branch, P.O. Box 5020, St. Louis, MO 63115-0020, via website at www.va.gov, or via telephone at (800) 827-1000.
Medical records for Marines discharged from active duty or from the active/inactive reserves after January 1, 2014 should be in the possession of the U. S. Navy Bureau of Medicine and Surgery (BUMED), Navy Records Activity (NMRA). You may contact that agency by writing to them at:Navy Medicine Records Activity, BUMED Detachment St. Louis, 4300 Goodfellow Drive, Bldg. 103 St. Louis, MO 63120.
Medical treatment received at civilian medical facilities should be requested from that perspective facility.
